CONTEST PERIOD 

November 8 to December 6, 2017

ELIGIBILITY

Participants must be 18 years or older, or accompanied by a parent or guardian.   One email account may be used for multiple family members, and all medical history for each family member should be completed (“My Health” page).

All participants 18 years of age and older must agree to ezMedicall’s Terms of Use and Privacy Policy by completing the Test Phase form below.   

PRIZES

First Prize (Android smartphone), estimated value IDR 3.000.000

Second Prize (Wearable activity monitor), estimated value IDR 2.000.000

Third prize (4G wifi modem), estimated value IDR 500.000

Other prizes (10 prizes for voucher facial/body treatment), value IDR 100.000 each.

**ezMedicall reserves the right to modify the prizes, although the estimated value will remain the same.

PROCEDURE 

STEP 1.        Complete the Test Phase form below.

STEP 2.        Install the app to your mobile device (links below) and/or open ezMedicall from your web browser* (link below).  Then, register as a patient by 7 November.

STEP 3.       Complete your Health Profile by 7 November

After registering on the website or mobile app, login and complete the pages in “My Health”.

To add additional family members, go to “My Family Profiles”, and click “add Family Member” and complete the information for that person.   After your family members have been added, clicking on a photo of any family member will open that family member’s health portal and that person’s “My Health” page can be completed.

WINNER SELECTION (POINT SYSTEM)

I.     Number of sessions

Each participant receives 20 points for a completed session.  If a session is cancelled due to:

  • Poor internet connection (before the session),
  • Poor internet connection (within the first three minutes of the session), or
  • The doctor is not available due to emergency or other reasons

…then no points are awarded for that session.

II.     Quality of suggestions and feedback

Each participant is required to complete the on-line feedback form after each session.    Points are awarded as follows:

5                 Grammatical errors

10               Minor issue

20               Major issue:

i.     Any situation where the app crashes,

ii.     The app action reports an incorrect result,

iii.    There is a process flow problem

Also, 20 points will be awarded for any new suggestions that ezMedicall decides to implement in version 2.0.   Please review Section 8.3 (Submissions to the App) in the Terms of Use.

**Points can also be earned when the user is not in a session with the doctor.  For example, issues found in the user’s personal portal qualify for points.**

During the entire 4-week test, participants are awarded points one time for each unique identified issue.   No points are awarded for duplicate problems.

III.     Tie-breaker

In the event that two or more participants have the same number of total points at the end of the Contest, the deciding factors (in order of consideration) will be based on:

i.     The participant with the highest number major bugs identified.

ii.    If the number of major bugs for two candidates is the same, the next deciding criteria will be the the participant with the highest number of new suggestions.

iii.   If there is still no decisive winner, the participant with the highest number of minor bugs identified will be the winner.

iv.   If there is still no decisive winner, ezMedicall’s judging panel will vote to decide the winner.  Voting will be based on the participant that submitted the suggestion that, in the judges opinion, is most likely to have the greatest positive impact on the next version of the app.

WINNER NOTIFICATION

ezMedicall will notify the winners by December 17, 2017.   Winners will also be published on our website and social media pages.

OTHER INCENTIVES

CASH AWARD FOR BEST SUGGESTIONS 

Cash awards will be paid to participants with the top three suggestions:   IDR 300.000, IDR 200.000 and IDR 100.000.   Please review Section 8.3 (Submissions to the App) in the Terms of Use.

TRAINING

Participants will be asked to attend a brief 45 to 60-minute training session at our office:  Jl. A.M. Sangaji No. 33, Jakarta Pusat (area Harmoni).   Participants can choose one of the available dates below:

November 7 (Tuesday)

November 8 (Wednesday)

Training is not mandatory but is recommended to improve your chances of winning.  The time for the training will be during normal office hours, and will include snacks and drinks.   All participants that attend will also receive a souvenir.

ADDITIONAL INFORMATION

ezMedicall reserves the right to disqualify a participant if there is suspicion of ineligibility or fraud.

ezMedicall (and its sponsors) reserve the right to terminate, suspend or amend the Contest, without notice, and for any reason, including, without limitation, if ezMedicall determines that the Contest cannot be conducted as planned or should a virus, bug, tampering or unauthorized intervention, technical failure or other cause beyond our control corrupt the administration, security, fairness, integrity or proper play of the Contest. In the event any tampering or unauthorized intervention may have occurred, ezMedicall reserves the right to disqualify suspected participants.  ezMedicall, its sponsors, and any telephone network or service providers, are not responsible for incorrect or inaccurate transcription of entry information, or for any human error, technical malfunction, lost or delayed data transmission, omission, interruption, deletion, line failure or malfunction of any telephone network, computer equipment or software, the inability to access any website or online service or any other error, human or otherwise.
